Consent Declaration of the Pet Owner

This information is based on the Spanish law Ley Orgánica de Protección de Datos (LOPD) and represents an approximate translation. Even after the implementation of the GDPR, explicit consent from the pet owner for the processing of their personal data is still not required, provided that the data collection is necessary to fulfill the veterinary treatment contract (Article 6(1)(a) GDPR).

The following data is included:

• Pet owner's name
• Address
• Date of birth
• Payer
• Treatment dates
• Provided services
• Medications and diagnoses

The entry into force of the GDPR does not change the existing legal documentation and retention obligations, as according to Article 17(3)(b) GDPR, deletion obligations do not apply if processing is necessary to fulfill a legal obligation. Therefore, for example, the retention regulations for tax-relevant veterinary documentation remain valid. Professional regulations on documentation and retention obligations also constitute an appropriate legal basis under the GDPR.

A deletion request from the pet owner can be opposed based on the existing legal norms for retention. After the retention periods expire, the aforementioned data will be deleted. The pet owner has a legal right to have their data deleted thereafter.

By submitting the form, you give your consent for your personal data to be shared/used as follows:

• With billing offices
• With other veterinarians or clinics for further treatment
• With laboratories and institutes for diagnostic purposes
• For receiving the practice newsletter
• For receiving vaccination reminders and communication about treatments
